Grad Student Forms
This form is for use by all graduate students who have not yet passed their preliminary oral exam. The form is due by the Friday of Week 1.
If you plan to take Individualized Study you do not need to fill out the form but you must send information about who the study is with so I can enter it.
Petition for Exemption to Sequence Requirement
This form may be used to grant an exemption to one of the sequence requirements.
Post-Qual Advising Form (“Orals Plan”)
During the spring term after you pass your qualifying exams, you will need to choose an advisor and work with them to complete this form.
Use this form to apply for a master’s degree (MS). You must Apply for an Advanced Degree in GradWeb by the second Friday of the term in which you would like to received the degree.
Language Exam Requirement Form
This form must be completed and turned in prior to your oral exams.
Committee & Syllabus Application
This form is to be turned in 30 days before the oral exam. It will list the department members of your dissertation committee along with their signatures. You will attach a syllabus of material that you plan to cover in your oral exam.
This form needs to be taken to the oral exam and given to the Graduate Coordinator immediately afterwards.
Use this form to update your dissertation committee to include the Institutional (Outside Member) Representative. This form must be turned in 6 months prior to your defense date.
